We've detailed the steps below to become a new patient at our practice. Because we take insurance and also offer patients the ability opt out of insurance, booking a new appointment at our office may look a little different than what you are used to. Please follow the steps below and reach out to us if you have any questions.

1. Watch Our Welcome Video (below)
Get to know what to expect at Endurance Mental Health & Wellness.
2. Schedule Your Initial Evaluation
Book your first appointment online to begin your care journey. (Calendar link at bottom of this page)
3. Complete All Intake Forms Promptly in Charm Health.
Follow the instructions emailed to you after booking. Please fill out all questionnaires in the Charm Patient Portal as soon as possible — we need time to review them before your appointment.
4. Decide How You Will Pay for Services
You may choose to be a self-pay patient or use commercial insurance through: United Healthcare, Anthem Blue Cross Blue Shield, Cigna, Aetna, Carelon Behavioral Health, or Optum.

5. Using Insurance? Set Up Your Headway Account
If you're using insurance, please create your Headway account right away — verification can take time.
Self pay clients can skip this step!
6. Get ready for your appointment!
Our office will be in touch soon. Please collect any past medical records, Genesight testing, past medication lists, or other pertinent health data to bring with you to your appointment.
Note Our Cancellation Policy
We require 48 hours’ notice for any cancellations or rescheduling.
Late changes will result in a $150 fee.
