Free 15-Minute Phone Consultation

Service Description

•Once you fill out the inquiry form, we will call you within 48 business hours to scheduled your free 15-minute phone consultation. •We will go over your goals for treatment, get a brief psychiatric history, and answer any questions you may have about our practice. If both parties agree to proceed with the relationship, then we will pick a time for the formal initial evaluation. •Once you have completed the free phone consultation, we will send you a link to set up your initial evaluation. This will include new patient paper work, HIPPA forms, and policies that must be signed prior to your appointment. Please have your documents uploaded 24 hours prior to your appointment (By 5pm Friday for Monday appointments). •We will also need a credit card on file prior to confirming the appointment to save your appointment time. •Most initial evaluations can be accommodated within 1-2 weeks of inquiry.
